The Adult ADHD Self-Report Scale (ASRS v1.1) is a 6 to 18-item self-report questionnaire specifically developed to evaluate symptoms of Attention Deficit Hyperactivity Disorder (ADHD) in adults aged 18 and above. Please read each statement and select an answer to indicates how much the statement applied to you over the 6 months.

There are no right or wrong answers. Please answer every question as honestly as possible. If you feel you need to talk to our mental health professionals, remember that we are here for you.
